Mouth Ulcers: When Should You Worry?
Most people experience a mouth ulcer at some point in their lives. In many cases, these small sores are harmless and heal on their own within a week or two. They may occur after accidentally biting the inside of the cheek, eating spicy foods, or due to stress. However, not every mouth ulcer is harmless. A mouth ulcer that does not heal, keeps returning, or is associated with other symptoms may be an early warning sign of oral (mouth) cancer. Knowing when to seek medical attention can lead to early diagnosis and better treatment outcomes.
What Are Mouth Ulcers?
Mouth ulcers are small sores that develop on the inside of the lips, cheeks, gums, tongue, or roof of the mouth. They are usually painful and may make eating or speaking uncomfortable.
Most common mouth ulcers are non-cancerous and heal naturally.
Common Causes of Mouth Ulcers
Many everyday factors can lead to mouth ulcers, including:
➜ Accidental bites or injury inside the mouth
➜ Sharp or broken teeth
➜ Ill-fitting dentures
➜ Stress and lack of sleep
➜ Vitamin B12, iron, or folic acid deficiency
➜ Spicy or acidic foods
➜ Certain viral infections
These ulcers typically improve within 7–14 days.
When Should You Be Concerned?
A mouth ulcer should never be ignored if it:
Lasts More Than Two Weeks
Any ulcer that does not heal within two weeks should be examined by a healthcare professional.
Continues to Grow
An ulcer that becomes larger instead of healing requires prompt medical evaluation.
Bleeds Easily
Persistent bleeding without obvious injury can be a warning sign.
Is Painless
Although many harmless ulcers are painful, some early oral cancers may be painless, making them easier to overlook.
Is Associated With a Lump
A lump in the mouth, jaw, or neck together with a mouth ulcer should be assessed immediately.
Causes Difficulty Swallowing or Speaking
Persistent difficulty swallowing, chewing, or speaking should not be ignored.
Who Is at Higher Risk?
Certain factors increase the risk of oral cancer, including:
➜ Smoking cigarettes or bidis
➜ Chewing tobacco or gutkha
➜ Excessive alcohol consumption
➜ Poor oral hygiene
➜ HPV (Human Papillomavirus) infection
➜ Long-term sun exposure affecting the lips
➜ Age over 40 years
People with these risk factors should be especially vigilant about persistent mouth ulcers.
How Is Oral Cancer Diagnosed?
If your doctor suspects something more serious, they may recommend:
➜ A detailed examination of the mouth
➜ Imaging tests if required
➜ A biopsy (removing a small tissue sample for laboratory examination)
A biopsy is the most reliable way to determine whether an ulcer is cancerous.
Can Oral Cancer Be Prevented?
Many cases of oral cancer can be prevented by making healthy lifestyle choices:
➜ Avoid all forms of tobacco
➜ Limit alcohol consumption
➜ Maintain good oral hygiene
➜ Eat a balanced diet rich in fruits and vegetables
➜ Visit your dentist regularly
Seek medical advice for any ulcer lasting longer than two weeks
When Should You Consult a Specialist?
Consult a specialist promptly if you have:
➜ A mouth ulcer lasting more than two weeks
➜ A sore that repeatedly returns in the same place
➜ Persistent pain or bleeding
➜ White or red patches inside the mouth
➜ Difficulty swallowing
➜ A lump in the neck or mouth
➜ Unexplained weight loss along with mouth symptoms
Early evaluation can make a significant difference if the cause is serious.
Most mouth ulcers are harmless and heal naturally. However, an ulcer that persists beyond two weeks should never be ignored. Early diagnosis is one of the most effective ways to improve outcomes if oral cancer is present.
If you notice a persistent mouth ulcer or any unusual changes inside your mouth, consult a specialist without delay. Timely evaluation can provide reassurance—or ensure that treatment begins as early as possible.
