Persistent Cough: When Should You Get Checked?

A cough is one of the body's natural ways of clearing irritants, mucus, or infections from the airways. Most coughs are temporary and improve as the underlying cause gets better. However, a cough that persists, keeps returning, or is accompanied by other concerning symptoms should not be ignored. While persistent cough is often caused by conditions such as allergies, asthma, acid reflux, or infections, it can occasionally be associated with more serious diseases, including lung cancer. Knowing when to seek medical attention can help identify the underlying cause early and ensure appropriate treatment.

How Long Is Too Long for a Cough?
The duration of a cough can provide important clues.
Acute cough: Usually lasts less than 3 weeks and is commonly associated with infections such as a cold or flu.
Persistent cough: A cough lasting more than 3 weeks deserves medical evaluation, particularly if it is not improving.
Chronic cough: A cough lasting more than 8 weeks in adults is considered chronic and should be investigated.
The appropriate evaluation depends on your age, symptoms, medical history, and risk factors.

What Are Common Causes of a Persistent Cough?
A long-lasting cough does not necessarily mean cancer. Common causes include:
➜ Asthma
➜ Allergies or post-nasal drip
➜ Acid reflux
➜ Smoking or exposure to second-hand smoke
➜ Chronic respiratory infections
➜ Certain medications
➜ Chronic lung conditions
➜ Tuberculosis and other infections
Identifying the underlying cause is important because treatment depends on what is causing the cough.

When Should You Be Concerned?
A persistent cough deserves particular attention if it is accompanied by:

Coughing Up Blood
Blood in your sputum, even in small amounts, should be evaluated by a healthcare professional.

Unexplained Weight Loss
Losing weight without trying, especially when accompanied by a persistent cough or reduced appetite, warrants medical assessment.

Persistent Chest Pain
Chest discomfort or pain that continues or becomes worse when coughing or breathing should not be ignored.

Breathlessness
New or worsening shortness of breath along with a persistent cough requires medical evaluation.

Persistent Hoarseness
A cough accompanied by ongoing voice changes or hoarseness may need further investigation, particularly if it does not improve.

Recurrent Chest Infections
Repeated episodes of bronchitis or pneumonia may require investigation to determine why they keep occurring.

Unexplained Fatigue or Loss of Appetite
Persistent tiredness or a significant loss of appetite together with other respiratory symptoms should be discussed with a doctor.

Can a Persistent Cough Be a Sign of Lung Cancer?
Yes, a persistent cough can sometimes be a symptom of lung cancer, but it is important not to assume that every long-lasting cough is cancer.
Lung cancer may cause symptoms such as:
➜ A cough that doesn’t go away or changes over time
➜ Coughing up blood
➜ Chest pain
➜ Breathlessness
➜ Persistent hoarseness
➜ Unexplained weight loss
➜ Loss of appetite
➜ Persistent fatigue
➜ Repeated respiratory infections
These symptoms can also occur with many non-cancerous conditions. The only way to determine the cause is through appropriate medical evaluation.

Who May Have a Higher Risk of Lung Cancer?
The most important risk factor is tobacco smoking. Risk can also be increased by:
➜ Long-term exposure to second-hand smoke
➜ Certain occupational exposures
➜ Exposure to substances such as asbestos
➜ Certain environmental exposures
➜ A personal history of lung disease
➜ A family history of lung cancer in some circumstances
People with significant risk factors should discuss their individual risk and whether screening is appropriate with their doctor.

How Is a Persistent Cough Evaluated?
Your doctor may begin with a detailed medical history and physical examination.
Depending on your symptoms and risk factors, investigations may include:
➜ Chest X-ray
➜ Blood tests
➜ Sputum examination
➜ CT scan of the chest
➜ Lung function tests
➜ Bronchoscopy or other tests when necessary
Not everyone with a persistent cough needs every test. Your doctor will recommend investigations based on the suspected cause.

Can Persistent Cough Be Prevented?
Not every cause can be prevented, but several steps can help protect your respiratory health:
➜ Avoid smoking and tobacco products.
➜ Avoid exposure to second-hand smoke.
➜ Reduce exposure to occupational dust, fumes, and chemicals.
➜ Maintain good indoor air quality.
➜ Manage allergies, asthma, and acid reflux appropriately.
➜ Eat a balanced diet and stay physically active.
➜ Seek medical attention for persistent or worsening symptoms.

When Should You See a Doctor?
Consider getting evaluated if:
➜ Your cough lasts more than 3 weeks.
➜ The cough is getting worse instead of improving.
➜ You cough up blood.
➜ You experience unexplained weight loss.
➜ You have persistent chest pain.
➜ You develop breathlessness.
➜ You have persistent hoarseness.
➜ You repeatedly develop chest infections.
➜ You have a significant history of smoking or other lung-cancer risk factors.
Don’t wait for symptoms to become severe before seeking advice.

A persistent cough is often caused by a treatable condition, but it should not be dismissed when it continues or is accompanied by warning signs. Early medical evaluation can help identify the cause, provide appropriate treatment, and, when necessary, detect serious conditions at an earlier stage.
